The Closure of Provo Canyon School

On July 7, 2026, the Utah Department of Health and Human Services officially revoked the license of Provo Canyon School’s Springville campus—a psychiatric residential treatment facility for youth ages 12 to 18. The center must cease operations by August 6 and is barred from reapplying for a new license for five years.

Allegations and Regulatory Failures

The state’s decision followed a series of noncompliance findings dating back to 2025, including failure to maintain adequate staff‑to‑client ratios, neglect of medical emergencies, improper restraints and aggressive physical contact with clients, and lapses in background check protocols. These violations culminated in temporary restrictions in May, after staff delayed necessary medical treatment for a teen with serious injuries.
